Preschool Screening
Welcome to Preschool Screening!
The information gathered in this form and from the "Ages and Stages" preschool screener, will be used to determine your child's eligibility for enrollment, and/or whether additional assessments are necessary. Please include as much detail as possible when answering the questions. This form must be filled out in its entirety, including the "Permission to Screen Section", and required documents in Section 10 must be received by our office, in order to receive the link to the "Ages and Stages" preschool screener. Preschool screening happens year-round. Please visit our school calendar for screening dates. Children should be 3 by September 1st to be eligible for screening. If at any time you have concerns about your child's development and your child is 3 years old, please contact our office at 847-270-9920.
